Registering with the practice and having a named GP
You can register with us by either coming to the health centre and filling out a registration form, or downloading the form, completing it at home and bringing it to the surgery.
It is not a legal requirement to show ID to register as an NHS patient, however, there are practical reasons why we might need to be assured that people are who they say they are, or to check where they live. Seeing some form of ID will help to ensure the correct matching of a patient to the NHS central patient registry, thereby ensuring any previous medical notes are passed onto us. If you intend to register with us, please could you bring with you one of the following: A passport or driving licence and a proof of address, such as a utility bill showing your current address, or your current NHS registration card.

All patients are allocated a named GP but you are always welcome to see any GP of your choice.
